Understanding the tear trough

How tear trough filler works

Soft, low-viscosity hyaluronic acid fillers are injected beneath the tear trough depression to restore volume and reduce the appearance of hollowing. The filler lifts the overlying tissue, reducing shadows and creating a smoother transition between the lower eyelid and cheek.

Product selection is critical in this area. Fillers that are too firm or highly cross-linked can create visible lumps or a bluish discoloration called the Tyndall effect. Swiss practitioners typically use specifically formulated under-eye products with low G' values and minimal water-attracting properties.

Treatment is highly conservative—small amounts of filler (often 0.3 to 0.5ml per side) can produce significant improvement. Overcorrection in this area creates puffiness and an unnatural appearance.

Patient selection and assessment

Careful patient selection is essential for tear trough treatment. Ideal candidates have:

True hollowing: Volume loss creating a visible depression

Good skin quality: Reasonable thickness and elasticity

Minimal puffiness: Bags or significant fat herniation may worsen with filler

Realistic expectations: Understanding that improvement, not perfection, is the goal

Patients with significant fat bags, very thin or crepey skin, or primarily pigmentation-based dark circles may not be good candidates for filler. Alternative treatments such as blepharoplasty, skin tightening, or topical therapies may be more appropriate.

The treatment experience

Tear trough treatment requires precision and experience. The procedure typically takes 20 to 30 minutes.

After cleansing the area, topical anesthesia is applied to maximize comfort. Some practitioners use nerve blocks for additional numbing. The skin is very thin in this area, so patients may feel more sensation than in other treatment areas.

Injection technique is crucial. Most experienced practitioners use cannulas rather than needles for tear trough treatment—the blunt tip reduces bruising risk and allows precise placement with fewer entry points. Product is deposited deep, at the periosteal level, to minimize visibility through the thin skin.

Very small amounts are placed incrementally, with assessment between injections. It is always better to under-treat initially and add more at a follow-up appointment than to overcorrect.

Recovery and results

Swelling and bruising are common after tear trough treatment and may last one to two weeks. The thin skin and vascular nature of the area makes this region more prone to bruising than other facial areas.

Patients should avoid blood thinners, alcohol, and vigorous exercise before and after treatment to minimize bruising. Sleeping with the head elevated for the first few nights can reduce swelling.

Initial results are visible immediately but are obscured by swelling. Final results are best assessed at two to three weeks post-treatment. Improvement includes reduced hollowing, softer appearance of dark circles, and more refreshed, rested appearance.

Results typically last 9 to 12 months, though some patients experience longer duration. The area has relatively little movement, allowing filler to persist well. However, some patients metabolize filler quickly in this area and may need more frequent touch-ups.

Risks and complications

Tear trough treatment carries higher risk than many other filler areas due to anatomical complexity. Potential complications include:

Tyndall effect: Blue discoloration visible through thin skin, requiring dissolution with hyaluronidase

Lumps or irregularities: Visible bumps that may require massage or dissolution

Swelling or puffiness: Overcorrection or inappropriate product choice can worsen appearance

Vascular complications: Though rare, the periorbital region has important blood vessels requiring careful technique
